Forms can be submitted in the following manner: 1) Please fill out the application, print and return to your child's classroom, office or school cafeteria 2) Print out and mail to Food Service Department C/O Tahoe Truckee Unified School District, 11603 Donner Pass Road, Truckee, CA, 96161 3) Completed and secure faxed to 582-2527 In all cases, the form must be signed prior to submission.
